Patient and Family Advisory Council Seeking Nominations
Vanderbilt Health One Hundred Oaks
The goal of Vanderbilt Health One Hundred Oaks is to offer innovative ways for patients to receive healthcare, become healthy and stay healthy. One of the strategies by which we hope to accomplish this is through the creation of a dedicated consumer group called a Patient and Family Advisory Council. This group would collaborate with 100 Oaks leadership in planning and evaluating services, programs, policies, and teaching materials.
We are asking that you nominate patients and/or their family members who you think could make a meaningful contribution to this council. This is a volunteer council that will meet approximately once a month. The nominees must:
- have experience in a clinic that is relocating to 100 Oaks
- have time available to contribute to the council, either by attending monthly meetings and /or by providing input on projects in some capacity
- have a commitment to quality healthcare at Vanderbilt
Please send nominations and contact information to Terrell Smith, Director, Patient and Family-Centered Care (Terrell.Smith@Vanderbilt.Edu).
We hope to have the council members identified and oriented to the council by December, 2008
