If you did not get your pledge card during classroom training or have misplaced it,
simply print this page and complete the information below. Please be sure that you
have carefully read the Standards of Conduct and have had any questions concering
the Standards answered by your supervisor or someone in the Office of Compliance.

Pledge Cards need to be signed EVERY year. This is a change from the previous
policy. If you have access to a computer and can use our web-based training system (webinservice.com/vanderbilt/) please do. Use the Pledge Card below ONLY if you are
not able to access the web-based training.

These instructions are only for classroom training sessions. Please use the web-based
training (WBT) portal at http://webinservice.com/vanderbilt/. When using the WBT you
DO NOT need to complete the Pledge Card below and you DO NOT need to FAX it to us. Completing the SOC on-line is equivalent to signing a Pledge Card and is the preferred
method for meeting this requirement.

Print your Last Name, First Name, (Clearly Please) put your Department and
sign and date and drop this page into Campus Mail - Office of Compliance 3100
Village at Vanderbilt 8200. You can also send it to us through the US Mail. Office of Compliance 1500 21st Ave. South - Room 3100 - Nashville, TN. 37212 or Fax it to 343-9138.
