| Responsibilities of Those Making a Request |
Hello!
In order to help you, and those working on the problems you report, it's important for us to gather sufficient information. We will ask you questions because this is a collaborative process that helps both of us develop the most useful information about your problem to enhance our problem resolution process.
Whenever possible, please contact the Help Desk while you're in front of the affected equipment.
We'll need the following information to describe and document your problem:

Your complete contact information (User ID, first and last name, department name, building name, room and phone number). |

The V11, LMS ID, or Departmental number of the affected equipment (most terminals and printers should be labeled with this identifier). |

For issues involving Network connectivity, please provide as much detail as possible about the problem as well as an IP address or segment number (most terminals and equipment are labeled with this information). If the IP address is not readily available, we'll help you get it. |

For issues involving specific patients, please be prepared to provide the complete patient name and medical record number. |

A clear and specific description of the problem or request. |
|
When equipment is involved, we may ask you to check connections, power devices off or on, gather additional information, and otherwise help us develop detailed and more complete information. |
