Petitions for Special Elective Credit

Students should discuss proposed coursework with the faculty member who will supervise and evaluate their performance.  After completing the form and writing a description of the proposed coursework, the preceptor must sign the petition.  This form is submitted to the Office of Student Records to be presented to the Assistant Dean for Medical Student Affairs.

Drop/Add Form

To drop or add a course print this form and obtain signature approval from course director. Fourth year student must make changes at least one month in advance prior to the clerkship.

Student Leave Pass

Third and fourth year students on clinical clerkships wishing to be away from their rotation must print and complete this form.  The signature of the clerkship director is required and submission of form to the Office of Student Records for final approval from the Assistant Dean for Medical Student Affairs.   

Change of Address

Clinical Students

Currently enrolled students should update local address information using the Student Portal. Open link "Registration" (on the left navigation menu) to update or change local address information only!

Permanent Address/Parent 1 and Phone Number

For graduate and professional students, this is the address of a person who can always contact you. To update this information, student can print Change of Address Form and return to the Office of Student Records/Registrar's Office, 203 Light Hall or come by the Office and complete form.
