Changes in use of carbapenem class of antimicrobials announced

VUMC Pharmacy and Therapeutics recently reviewed the carbapenem class of antimicrobials and the following changes have been approved:

At the adult hospital, meropenem and ertapenem will be the formulary carbapenems. Doripenem and imipenem-cilastin will be non-formulary. All orders for doripenem will be automatically converted to the approved equivalent dose of meropenem by the pharmacist staff. This conversion will be notated in the comments section of the drug order when it occurs. Meropenem doses will routinely be administered over three hours.

At the Monroe Carell Jr. Children's Hospital at Vanderbilt, meropenem will remain the formulary carbapenem. Doripenem, ertapenem, and imipenem-cilastin will be non-formulary.

These changes will be implemented beginning Thursday, April 19.
