Anesthesiology

To apply to the Vanderbilt University Multidisciplinary Pain Medicine Fellowship, please complete the Application for Training. Please also provide:

• Official Transcript from medical school
• Official Transcripts of USMLE/NBME scores
• CV
• Dean’s Letter and 3 additional letters of recommendation
• Official licensure verification from all states in which you have been or are currently licensed
• Written explanation of any gaps in training, greater than one month, since graduation from medical school
• ECFMG certificate (if applicable)

Note: Prior to matriculation, all fellows will be required to provide a certificate of completion from an ACGME-accredited residency.
