 Evaluations

A. Evaluations of the resident will be completed by the attending and other CPS staff during and at the
    conclusion of the rotation.

B. Evaluations of the rotation are available through the New Innovations website at

     https://www.new-innov.com/Login/Login.aspx

C. Each resident shall maintain a personal procedure log to be reviewed by the attending at the end of
    each week.
