Standards of Conduct Pledge Card
If you did not get your pledge card during classroom training or have misplaced it,
simply print this page and complete the information below. Please be sure that you
have carefully read the
Standards of Conduct and have had any questions concerning
the Standards answered by your supervisor or someone in the Office of Compliance.

Pledge Cards need to be signed EVERY year.
This is a change from the previous policy. If you have access to a computer and can use our web-based training system(http://www.mc.vanderbilt.edu/root/vumc.php?site=The%20Learning%20Center) please do. Once you arrive at this page choose the quick link entitled "The Learning Exchange" and log onto the system using your VUNetID and password.  Use the Pledge Card below ONLY if you are not able to access the web-based training.

 

Print your Last Name, First Name, (Clearly Please) input your Department and sign, date and drop this page into Campus Mail for delivery.
