General Surgery (Oncology/Endocrine) (R4) - VUMC

General Surgery - OES - R4

Medical Knowledge and Practice-based Learning and Improvement:
Upon completion of the rotation, the senior resident should demonstrate understanding and  competence in the surgical care of patients with gastrointestinal malignancies and other complex surgical oncology diseases. This includes a detailed knowledge of the pathophysiology, staging, prognosis, and multi-modality treatment of common solid tumors. Familiarity with the literature regarding surgical management of malignancy including areas of controversy is also expected.
 
Technical Skills:
Upon completion or the rotation, the senior resident demonstrate proficiency with a broad variety of minimally invasive and open oncologic procedures including esophagectomy, gastrectomy, pancreatico-duodenectomy, distal pancreatic resections, hepatic resection, resection of retroperitoneal tumors, complex colorectal oncologic procedures, splenectomy, and adrenalectomy.
 
Communications and Interpersonal Skills--Other Attributes of Resident:
The senior resident should instruct medical students and residents about the preoperative and postoperative care of surgical patients and the principles of surgery. They should also demonstrate the ability to teach junior house staff basic surgical skills and assist them with introductory level cases. The senior resident must display leadership skills and the ability to run an effective multi-level service in conjunction with the Chief resident with numerous residents and medical students on the service. Organization, time management and administrative skills for managing a team of residents and students should be demonstrated.
 
Patient Care and Technical Skills Benchmarks:
-Be proficient in  major gastrointestinal oncologic procedures (open and minimally invasive): Esophagectomy, gastrectomy, pancreatoduodenectomy, distal pancreatectomy, hepatic resection, splenectomy, resection of retroperitoneal and pelvic tumors and colorectal malignancies.
-Be proficient in the complex endocrine surgical procedures: Reoperative thyroidectomy and parathyroidectomy, adrenalectomy.
-Be knowledgeable and conversant in the evaluation and management of the patient with cancer. Participate actively in the VUMC Multidisciplinary Solid Tumor Conference.
 
Milestones including Systems-based Practice:
-Verbally describes process of Pancreaticoduodenectomy and distal pancreatectomy
-Understands w/u and treatment of Pancreatic Mass/Cysts
-Understands w/u and treatment of Esophageal and Gastric CA
-Understands w/u and treatment of primary, recurrent and metastatic colorectal CA
Understands w/u and treatment of retroperitoneal sarcomas
Understands w/u and treatment of primary and metastatic neuroendocrine tumors
 
Evaluation: Faculty and Chief residents will evaluate the performance of each senior resident using these goals and expectations. In turn, each resident will evaluate rotation faculty and Chief residents as well as the service itself.
