Project Description: The development of numerous tools that will foster greater patient engagement and interaction across a set population of patients. Unlike current tools, capabilities fostered through the MHT@V model would be patient centric. That is, ensure patient activation and engagement, communications around patient choice and preferences, and measure adherence and execution of care plan. Patient centric capabilities will enhance system coordination and efficient use of resources while delivering personalized care.
