Acknowledgements

Please note that all TPSR Forms contain an Acknowledgement Agreement that reads as follows:

"I understand that I am expected to acknowledge the contribution of the Translational Pathology Shared Resource and CHTN in all publications resulting from the use of these tissues and additionally forward all references citing this grant to the TPSR or CHTN as appropriate."

Comparative Pathology Forms

Comparative Pathology forms can be submitted in the following way:

•         Emailed to TPSR-Comparative_Pathology@vanderbilt.edu

   1)  Comparative Pathology Core Submission Form

Human Tissue Acquisition Forms

Human Tissue Acquisition forms (TPSR and CHTN versions) must be submitted in the following way and provide a 24-hour notice prior to tissue acquisition:

•        Emailed to TPSR-Tissue_Acquisition@vanderbilt.edu

   1)  Histology Request for Tissue Acquisition

   2)  Request Form for Prospective Tissue Collection - Consented or SPORES

   3)  Request Form for Prospective Tissue Collection - De-Identified 

 - CHTN Form must be submitted to TPSR-Tissue Acquisition

  4)  Request Form for Tissue Bank Withdrawals 

   5)  Request Form for Pulling Blocks and Slides

- All requests for Pulling Blocks and Slides must be submitted electronically to TPSR-Tissue_Acquisition@vanderbilt.edu.  Incomplete request forms will be returned to the sender until all information is supplied.
