This form should be completed by anyone who wishes to become a member or affiliate of the Vanderbilt-Meharry Center For AIDS Research.

This form should completed in order to access the services of the CFAR Clinical Discovery Core and Data and Specimen Repository.
Clinical Discovery Core-Project Update Form
This form should be completed every 6 months by those who have a active project with the Clinical Discovery Core.
