Health Record Program


Health Record Standards works under the guidance of the Health Record Executive Committee (HREC) at Vanderbilt University Medical Center (VUMC) who sets the strategy and guiding principles for the creation and use of the health record and is responsible for the continued migration and evolution of the health record.  The HREC is led by Dr. Titus Daniels and Gaye Smith and reports to the Medical Center Medical Board.  The Health Record Program Office team supports the work of the Health Record Program. 

The work is achieved through the Health Record Committee structure composed of subcommittees and workgroups, which include individuals from across the clinical enterprise.  Primary focuses include:

  • Policy creation to define standards for the use and functionality of the medical record based on best practice and regulatory guidelines
  • Deployment of new and revised policies
  • Migration of the organization to a fully electronic record through direct electronic entry and electronic accessibility of documents
  • Advancements to the Vanderbilt patient portal (MyHealth at Vanderbilt)
