Nursing Insertion Checklist

Numerous studies have documented the usefulness of standardization for reducing infections, especially when performing common high-risk procedures.  In order to improve aseptic technique during CVC insertion, all lines placed at VUMC should be documented using the Central Line Insertion Procedure Form in Star Forms.  This document notes adherence to process measure necessary to minimize catheter-related infections.  Providers are encouraged to familiarize themselves with this document.

Performance data generated from these forms will be available for providers.  Of note, only deidentified aggregate performance data is available—individual provider measures are never tracked.  These decisions were made to ensure honest reporting and encourage a blameless environment conducive to quality improvement.
