If you plan to teach outside of the Resuscitation Program, please complete the appropriate online course approval form:
ACLS Course Approval Form
BLS Course Approval Form
PALS/PEARS Course Approval Form

If you have slots in your class for others, we may be able to help fill it.

CHANGES: The online roster is no longer available. Please forward all rosters, written tests, skills check sheets and evaluations to the Vanderbilt Resuscitation Program. NOTE: Please remember all paperwork (excpet the written test) can be found on your instructor CD that comes with your instructor manual.
