VUMC Observational Experience

If you are a non-Vanderbilt undergraduate student, a graduate school student, a 1st, 2nd or 3rd year medical student or a working professional, and are interested in a career in health care, you will need to complete the following:

• Name and contact information of the health care professional you wish to observe
• Completed Health Screening Form
• A completed online application
• A copy of the front and back of your health insurance card
• Completed Online Safety Quiz
• Signed Confidentiality Agreement

Please submit all documents at the same time.  You may submit documents to  Corey Blaske via email at corey.blaske@vanderbilt.edu, or by faxing them to (615)343-5460.

Once we have all the required documentation, you will be notified of your acceptance status and observation dates.  If you have questions or need assistance, please contact Corey Blaske.
