Tube Type: Plain red top (No gel)
Specimen: Blood
Alternate Specimen: Red top (with or without gel), K2-EDTA, K3-EDTA
Pediatric Requirements: 1 dark green microtainer
Volume: 5 mL blood
Minimum Volume: 0.5 mL serum or plasma
Temperature: Refrigerated
Stability: Refrigerated: 7 days, Frozen at -10 C if stored longer than 14 days
Reasons for Rejection: Hemolysis, Lipemia, Icteric specimens; specimens containing RF, HAMA type 1-2 (human anti-mouse antibodies), and Total Protein, improper collection, QNS

Special Instructions: Indicate if peak or trough specimen. Peak serum levels generally - draw 30 minutes after 30 min IV - Submit: date/time IV start, date/time IV stop, peak collection date/time, dosage amount/interval Trough serum levels generally - draw 0-30 minutes before next dose, following third dose - Submit: date/time last dose charted, collection date/time, dosage amount/interval
