: Gross hemolysis

: Indicate if peak or trough specimen. Peak serum levels generally - draw 20 minutes after 30 min IV - Submit date/time IV start, date/time IV stop, peak collection date/time, dosage amount/interval. Trough serum levels generally - draw 0-30 minutes before next dose, following third dose. *Date/time last dose charted. Collection date/time. Provide dosage amount/interval.
