Assignments include three variations of the Take-A-Panel assignment (CPOE, Medical Errors & Practice Variability), the Share-A-Panel assignment, the two-part Synthesis assignment and finally the homework assignment.

Take-A-Panel - CPOE
An axiom that describes how creative work happens at the VCBH Innovation Center states: "You can't get there from here, but you can get here from there."

Take a moment to imagine what the state of CPOE utilization is likely to be in the year 2005. Create a response on your WorkWall panel to the questions asked below. Express your point of view from a perspective that is three years in the future from today in which you are looking back towards now and reflecting on what has happened.
Use more than one color to make clear distinctions on your panel. Write large and legibly. Incorporate diagrams, models or pictures to enhance the clarity of your ideas. You have 25 minutes.

Start by writing your name in the upper right hand corner of your panel. Then determine your point of view. For example, are you representing the perspective of a physician or other caregiver, a patient, a CMO, CEO or President, a CIO, an investor, payer, service provider or supplier?

Build a scenario of CPOE in 2005. Choose if it has been successful - or failed. Provide a measure of this success or failure.

Respond to these questions from that perspective:

  • What led to this outcome? What were the most critical drivers that produced this result?
  • What were the barriers within your control that had to be overcome? How was this done?
  • What barriers were outside of your direct control? How did you respond? To what degree were your successful?
  • What have been the cultural and political issues? How were they addressed?
  • Where has technology helped? Where has it been a hindrance? How well are people (providers, patients, payers, suppliers) accepting new technology?
  • How well are physicians accepting outside evidence now? What have been the critical requirements for them to do so? What has prevented them from doing so?
  • How well are caregivers accepting systemic processes now? What are the critical requirements for them to do so? What is still preventing them from doing so?
  • Describe the quality of the health care system. Has it improved or has it gotten worse? If better, why? If worse, why?

Share-A-Panel
Find your name on the list below and meet up with the rest of your team. Take five minutes for each person to report out their Take-A-Panel. Ask questions for clarification only. You'll have time for discussion later. Be aware that some of you may have had different Take-A-Panel assignments. When everyone has reported out there will be another assignment.



Synthesis (part a)
Take about 15 minutes as a team to dialog about the similarities and differences in what was stated in your panels, as well as what you consider to be the most compelling or salient issues. Choose one person to capture the conversation on your WorkWall.

These are some of the themes that you addressed:

  • Your perspective on the situation
  • The scenarios you created
  • Outcomes and drivers
  • Barriers within and outside of your control
  • Cultural and political issues
  • Usability and acceptance of technology
  • Acceptance of outside evidence
  • Acceptance of systemic Process
  • Quality of the healthcare system


You will receive a second part shortly.


Synthesis - (part b)

Develop a detailed response to the following issue:

  • How do you build TRUST?
  • How do you know what is trustworthy?
  • What is ACHIEVABLE now?
  • What are we ready to do? Which practices are proven and ready for transformation?
  • What BREAKTHROUGHS are needed in policy, reimbursement, caregiver roles, change methodology, processes and technology?
  • What BLOCKS need to be removed in policy, reimbursement, caregiver roles, change methodology, processes and technology?
  • What SHIFTS IN THINKING are essential (which sacred cows need to be eliminated) in policy, reimbursement, caregiver roles, change methodology, processes, technology?

For your report:
Prepare two HyperTiles; (see example) one that is your team's response to the issue above and another that captures your earlier synthesis.

You have 30 minutes to finish developing you content, prepare your tiles and a develop a complete 5 minute report.



Homework:
Over the course of the evening ponder what your ideal state would be. Given the best possible outcomes what do you expect to have achieved in the next three years? When you awaken in the morning, write your first thoughts about this on a note card.
