For Referring Providers

We can see your patient within 24 hours if urgent, but not life-threatening. Emergencies should go to the nearest ED. For any concerns, pediatric or adult, Monday through Friday between 8 am and 5 pm, please call 615-936-1765. For urgent calls concerning children after regular business hours, please call Pediatric Hematology at 615-936-1762. For urgent calls concerning adults after 5 pm, please call 615-936-1803.

Non-urgent and routine appointments can be seen in less than two weeks. We see adult patients on Tuesday morning and Wednesday mornings. Children are seen on Wednesday or Friday mornings. We ask that you fill out the attached form and fax it to the attention of "Hemostasis Clinic" at 615-936-1767. One of our Nurses will call back to confirm demographics, referral data and appointment time.
