    Joint Commission Readiness: Quality and Patient Safety Forum

    Be prepared -- attend an upcoming Joint Commission Readiness Forum

    Vanderbilt University Medical Center (VUMC) employees are invited to attend upcoming Joint Commission Readiness Forums hosted by the Accreditation and Standards team. Forums will be held the third and fourth Fridays of each month until the survey at the Strategy and Innovation Center located at 3401 West End Ave. Forums will provide participants with information to promote survey readiness.

    The daylong training will consist of four, mandatory morning sessions and a selection of breakout sessions that participants choose from in the afternoon. A total of 6.5 hours of continuing education credits will be provided.

    Training topics include:

    Morning Session

    • Environment of Care and Life Safety: This session will focus the most commonly identified environmental safety concerns, including corridor clutter, fire safety compliance and cleanliness. Additionally, the session will review VUMC's processes and policies and discuss whether there are current practices within areas that do not meet the VUMC-set standards.  
    • Infection Prevention: As one of the top nationally cited subjects, the Infection Prevention session will be crucial to addressing many central issues affecting patient safety and compliance. Topics will focus on the design and scope of VUMC's Infection Prevention Program, including: 
      • CAUTI
      • CLASBI
      • Disinfection and sterilization
      • Reprocessing single-use devices
      • Storage and disposal of medical equipment, devices and supplies 
    • Provision of Care, Record of Care: This session will identify and discuss care activities, including but not limited to:
      • Patients’ risks and interventions related to falls
      • The administration of moderate or deep sedation and anesthesia
      • Provision of blood and blood components
      • Comprehensive pain assessment
    • Tips and Survey 101: This session will focus on educating staff on who regulates VUMC. This may include but is not limited to:
      • Frequent citations/findings
      • Current enterprise regulatory risks
      • Events that trigger investigations
      • How Accreditation and Standards can assist you

    Afternoon Breakout Sessions

    • National Patient Safety Goals and Patient Rights: This session will identify and discuss current National Patient Safety Goals, with the specific focus on alarm fatigue. The training includes education on the new clinical alarm policy and customizing alarms based on individual patient needs. In addition, this session will discuss other National Patient Safety Goals, such as universal protocol, patient safety risks and patient identification. 
    • Medication Management: This session will focus on the construct of our safe medication management system which addresses our organization’s medication processes. This includes the following (as applicable to the individual group):
      • Planning
      • Selection and procurement
      • Storage
      • Ordering
      • Preparing and dispensing
      • Administration
      • Monitoring
      • Evaluation
    • What’s Hot/What’s Not: This session will focus on updates from The Joint Commission. The training will include information on The Joint Commission’s new scoring methodology the safer matrix. Individuals will get to use findings from the 2015 survey and apply to this methodology. Other topics covered will be information surrounding Sentinel Event Alerts #57 (developing a safety culture) and #58 (handover communication).
    • Ligature Risk Assessment: This session will be a workshop in identifying hazards in the environment that may lead to ligature risk. 
    • Sterilization and High Level Disinfection: This session will focus on identifying essential steps involved in each stage of reprocessing reusable instrumentation.
    • Human Resources: This session will focus on understanding The Joint Commission (TJC) Human Resources standards and how to organize and deliver documents during a TJC survey. This includes what type of information should be located in employee files.
