Medication Reconciliation Training: Deadline is today

Medication Reconciliation Training: Deadline Extended to Jan. 16

Last November, medication reconciliation training launched across Vanderbilt for most clinical professionals. Medical Center leadership has decided to extend the deadline for its completion to Jan. 16.

Medication reconciliation policy and associated Standard Operating Procedures (SOPs) have been updated, notably defining medication reconciliation as a process, not a single event, involving:

  1. Obtaining a Medication History,
  2. Review and decision-making by a healthcare provider, and
  3. Patient counseling about changes to the medication list.

Colleagues, based on their specific roles and responsibilities, have been assigned training modules within the Learning Exchange. Please log into the Learning Exchange and review “My Courses” to complete this training by the January 16 deadline. Th online modules are expected to take no more than 30 minutes to complete (many colleagues can complete in less than 20 minutes).
